BPC-157 has no approved indication. Off-label does not apply. There is no label to be off of. What exists is preclinical work, a thin early-human set, and anecdote. Do not average those into a glow.

Bottom line

Most of the literature is rodent, in vitro, or reviews of that series. The human papers in this set are small, uncontrolled, or both. They cannot confirm a general effect. There is no published RCT in this set. Unpublished trial registrations are not results.

Early human

PMID 34324435

Tested: retrospective intra-articular knee administration in mixed knee diagnoses; 16 of 17 people reached by phone follow-up. No control group.

Does not show: validated efficacy. Repair-of-tears and cartilage claims in the paper are not observed endpoints. Cannot confirm a general effect.

PMID 39325560 — Lee 2024

Tested: pilot in 12 women with interstitial cystitis who had not responded to pentosan polysulfate; Global Response Assessment after a single local administration during cystoscopy. No control group.

Does not show: a controlled effect, a male population, or a musculoskeletal claim.

PMID 40131143 — Lee & Burgess 2025

Tested: intravenous administration in two adults, with labs and vital signs before and after. Safety/PK-style observation. No efficacy endpoint.

Does not show: effectiveness for any condition. n=2 is not a general safety database.

Preclinical

PMID 14554208 — Staresinic 2003

Tested: transected rat Achilles tendon, plus tendocyte growth in vitro.

Does not show: a human tendon result.

PMID 21030672

Tested: tendon outgrowth, cell survival, and cell migration in laboratory systems.

Does not show: a human clinical effect.

Tested: reviews of rodent and in vitro BPC-157 work, sometimes with mention of a thin human series. 34267654 refers to ulcerative colitis and multiple sclerosis trials; those trials are unpublished and are not human evidence.

Does not show: a new controlled human study. A review of animals is still preclinical.

What is not established

A general healing effect in people. Controlled efficacy for knee pain, tendon, muscle, gut, or interstitial cystitis. Product identity between laboratory material and any vial in circulation. NCT07437547 is recruiting. NCT02637284 is unpublished. Neither is a result.
