Eloralintide

Selective long-acting amylin receptor agonist (LY3841136) — investigational obesity candidate.

Editorial review:
What evidence says

Evidence snapshot

This drawer marks papers. It is not a plan, a dose, or a source. A paper is a bounded report. It is not a protocol. Grades name the object. They are not a safety rating.

Identity lock. Eloralintide (developmental code LY3841136) is a selective, long-acting amylin receptor agonist (development literature emphasizes preferential AMY1R activity versus non-selective amylin analogues). PubChem CID 175663130; formula C201H319N49O65S2 / MW ~4526; CAS 2883634-40-8. It has no approved drug indication. Off-label does not apply. There is no label to be off of. It is not cagrilintide (a different long-acting amylin analogue, often studied with semaglutide as CagriSema), not semaglutide, and not a GLP-1/GIP dual or triple agonist. Identity confidence for the named clinical object is high on the primary Lilly trial reports below; broader cardiometabolic / phase 3 extrapolation remains thin. Always pair the name with LY3841136 in searches — bare LY queries explode.

Bottom line

PubMed holds a discovery-to–phase 1 proof-of-concept package and a 48-week phase 2 randomized obesity trial without type 2 diabetes. Those are investigational human objects. Secondary reviews and network meta-analyses discuss the amylin class; they do not create a label. No dedicated cardiovascular-outcomes trial for eloralintide appears in this set. Honest grade: therapeutic evidence is real but thin beyond phase 2 obesity.

Papers

Investigational — discovery / phase 1

PMID 41109426 — Briere 2025

Tested: discovery pharmacology of eloralintide (LY3841136) through preclinical models and a phase 1 single-ascending-dose proof-of-concept trial in healthy participants with overweight (randomized, placebo-controlled).

Does not show: confirmatory phase 2/3 labeled obesity treatment, or equivalence to cagrilintide.

PMID 41559929 — Bhattachar 2026

Tested: phase 1 proof-of-concept report for eloralintide as a selective, long-acting amylin receptor agonist for treatment of obesity.

Does not show: the 48-week phase 2 Billings object by itself, or an approved indication. (Related early-human package to Briere; mark as phase 1 POC, not phase 2.)

Investigational — obesity (phase 2)

PMID 41207310 — Billings 2025

Tested: once-weekly injectable eloralintide versus placebo for percent body-weight change over 48 weeks in adults with obesity or overweight plus a weight-related comorbidity, without type 2 diabetes (multicentre, double-blind, randomised phase 2).

Does not show: a labeled obesity indication, a type 2 diabetes population result, a CagriSema combination claim, or a dedicated CVOT.

Secondary / class context (not primary eloralintide efficacy)

PMID 42745233 — Sigalov 2026

Tested: literature review of emerging cardiovascular implications of selective amylin receptor agonism with focus on early eloralintide clinical signals.

Does not show: a cardiovascular-outcomes trial, or a labeled cardiometabolic indication.

PMID 41747885 — Bailey 2026

Tested: review of long-acting amylin-related peptides as therapies for obesity and type 2 diabetes (class context).

Does not show: a new eloralintide primary randomized result.

PMID 42452898 — Alhazmi 2026

Tested: review of amylin analogs as an emerging weight-loss therapy class, summarizing experimental data and early-phase clinical trials.

Does not show: confirmatory phase 3 eloralintide efficacy, or identity between eloralintide and cagrilintide.

PMID 42586227 — Fischer 2026

Tested: review of amylin-based pharmacotherapy beyond GLP-1 for weight-loss drug development.

Does not show: a new primary eloralintide trial object.

PMID 42175595 — Kamrul-Hasan 2026

Tested: network meta-analysis of novel amylin-based therapies for weight management in adults with overweight or obesity without diabetes.

Does not show: a head-to-head labeled claim; indirect comparisons are not single-trial proof for eloralintide alone.

What is not established

An approved obesity or diabetes indication. That eloralintide equals cagrilintide or CagriSema. A type 2 diabetes primary program result in this drawer. A dedicated major-adverse-cardiovascular-events trial. Product identity between laboratory or unlabeled material and the LY3841136 trial product. Phase 3 confirmatory evidence as a substitute for the phase 2 Billings object.
