Ipamorelin
Growth hormone secretagogue that acts at the ghrelin receptor.

Ipamorelin is a selective ghrelin-receptor / GHS-receptor agonist (GH secretagogue pentapeptide). It is not approved. Off-label does not apply. There is no label to be off of. Mark the object as GH-axis stimulation or the named clinical endpoint — not bodybuilding, fat loss, or “recovery.”
Bottom line
Human evidence is thin: a volunteer PK/PD infusion study showing episodic GH release, and one multicenter randomized proof-of-concept trial in postoperative ileus that did not meet its key efficacy endpoint versus placebo. Discovery and most mechanism work are rodent, swine, or in vitro. Do not average GH secretagogue pharmacology into a physique claim.

PMID 10496658 — Gobburu 1999
Tested: pharmacokinetic–pharmacodynamic modeling of ipamorelin infusions in healthy male volunteers; GH concentration responses across ascending infusion rates.
Does not show: a therapeutic indication, chronic use, body-composition change, or an oral product.
PMID 25331030 — Beck 2014
Tested: multicenter, double-blind, placebo-controlled phase 2 proof-of-concept trial of intravenous ipamorelin versus placebo for postoperative ileus after bowel resection (NCT00672074). Key endpoint: time to tolerance of a standardized solid meal.
Does not show: Did not meet key efficacy endpoint versus placebo (non-significant). Not a GH-deficiency treatment trial; not a body-composition trial.
Preclinical
PMID 9849822 — Raun 1998
Tested: discovery and pharmacology of ipamorelin as a selective GH secretagogue in rat pituitary cells, anesthetized rats, and conscious swine; ACTH/cortisol contrasted with other GHRPs.
Does not show: a human clinical effect beyond later volunteer/ileus work.
PMID 19289567 — Venkova 2009
Tested: ipamorelin in a rodent postoperative-ileus model (ghrelin-mimetic GI motility object).
Does not show: a human ileus success (see Beck 2014 for the human proof-of-concept result).
PMID 11735244 — Andersen 2001
Tested: ipamorelin versus glucocorticoid effects on bone formation markers in adult rats.
Does not show: a human osteoporosis or bone-building trial.
PMID 12168778 — Jiménez-Reina 2002
Tested: chronic ipamorelin in young female rats; somatotroph response studied in vitro afterward.
Does not show: a human pituitary or growth result.
PMID 15665799 — Adeghate 2004
Tested: ipamorelin-evoked insulin release from pancreas tissue of normal and diabetic rats.
Does not show: a human diabetes treatment claim.
Context, not a grade
Tested: narrative discussion of GH secretagogues and body composition in hypogonadal males — not an ipamorelin RCT.
Does not show: controlled ipamorelin efficacy for physique outcomes.
Doping-control and seized-product analytics that list ipamorelin among screened peptides are detection papers, not efficacy.
What is not established
Any approved use. Body composition, muscle, fat loss, sleep, or “recovery” as human treatment claims. Chronic outpatient GH-secretagogue therapy. Identity between volunteer/ileus trial material and any research vial. A failed or non-significant ileus primary analysis is not a green light for other uses.
