Bottom line
Tirzepatide has large, labeled human trials for type 2 diabetes (including ages 10 and older), for obesity, for obesity with type 2 diabetes, for weight-loss maintenance, for obstructive sleep apnea with obesity, and for cardiovascular outcomes versus dulaglutide in type 2 diabetes. MASH and HFpEF papers exist and are not those labeled uses. An EHR comparison is not an RCT of a labeled claim.

PMID 34170647 — SURPASS-2
Tested: tirzepatide versus semaglutide for glycemic control in type 2 diabetes.
Does not show: an obesity claim (that is SURMOUNT), or that the comparator product is the same object as tirzepatide.

Tested: tirzepatide versus placebo for body weight in adults with obesity, without diabetes.
Does not show: a diabetes-treatment claim, or a 3-year diabetes-prevention claim (see 39536238).
PMID 37385275 — SURMOUNT-2
Tested: tirzepatide versus placebo for weight in people with obesity and type 2 diabetes.
Does not show: weight effect in people without diabetes (that is SURMOUNT-1).
PMID 38078870 — SURMOUNT-4
Tested: continued tirzepatide versus withdrawal for maintenance of weight reduction in adults with obesity.
Does not show: what happens if a different product is substituted, or an unlabeled use.
PMID 38912654 — SURMOUNT-OSA
Tested: tirzepatide versus placebo for obstructive sleep apnea in adults with obesity.
Does not show: OSA treatment in people without the obesity enrollment, or a different product.
PMID 39536238 — SURMOUNT-1, 3-year
Tested: tirzepatide versus placebo over 176 weeks for weight in adults with obesity and prediabetes, and for onset of type 2 diabetes as a key secondary endpoint.
Does not show: a labeled diabetes-prevention indication. Weight in this population is the labeled object. Diabetes-prevention is not.
PMID 40353578 — SURMOUNT-5
Tested: tirzepatide versus semaglutide for weight in adults with obesity.
Does not show: that the two products are interchangeable for every use, or a non-weight claim.
PMID 41406444 — SURPASS-CVOT
Tested: tirzepatide versus dulaglutide for major adverse cardiovascular events in type 2 diabetes.
Does not show: a placebo-controlled CV claim, or CV effect in people without type 2 diabetes.
PMID 40975112 — SURPASS-PEDS
Tested: tirzepatide versus placebo for glycemic control in children and adolescents aged 10 to under 18 with type 2 diabetes inadequately controlled on metformin and/or basal insulin.
Does not show: an adult result (that is SURPASS-2), or an obesity-in-youth claim this paper did not test.

PMID 38856224 — SYNERGY-NASH
Tested: tirzepatide versus placebo for MASH with liver fibrosis.
Does not show: a labeled MASH use.
PMID 39555826 — SUMMIT
Tested: tirzepatide versus placebo for heart failure with preserved ejection fraction and obesity.
Does not show: a labeled HFpEF use.
Early human
Tested: observational EHR comparison of semaglutide versus tirzepatide for weight in adults with overweight or obesity.
Does not show: an RCT of a labeled claim. Assignment was not randomized.
What is not established
MASH and HFpEF remain unlabeled uses on this page. Diabetes-prevention on the SURMOUNT-1 3-year paper is not a labeled indication. Observational switching or comparison is not the trial object. A non-trial preparation is not SURPASS or SURMOUNT material.
